*WORK AUTHORIZATION –

I, the undersigned, am the owner/authorized representative of the premises at which the work mentioned above is to be done. I hereby authorize you to perform the Diagnosis/Solution and to use such labor and materials as you deem advisable. I agree to pay attorney’s fees and court costs in the event of legal action. If my check bounces, I understand that I could be liable for 3 times the amount of the check or $100.00. whichever is more, plus the face value of the check and court costs. I have read and agree to all terms and conditions set forth on the face side hereof, and have received a copy of the contract and notice to Owner (Section 7018.5 – Contractors License Law) stated below.

NOTICE TO OWNER –

This contract is registered with the State of Washington, registration no. INEEDNP878RQ as a general/specialty contractor and has posted with the State a bond or cash deposit of $12,000/$6,000 for the purpose of satisfying claims against the contractor for negligent or improper work or breach of contract in the conduct of contractor’s business. This bond or cash deposit may not be sufficient to cover claims which might arise from the work done under your contract. If any supplier or materials used in your construction project or any employee of the contractor or subcontractor is not paid by the contractor or subcontractor on your job, your property may be liened to force payment. If you wish additional protection, you may request the contractor to provide you with the original “lien release” documents from each supplier or subcontractor on your project. The contractor is required to provide you with further information about release lien documents if you request it. General information is also available from the Department of Labor and Industries.

ACCEPTANCE OF WORK PERFORMED –

I find the service and materials rendered and installed, in connection with the above work mentioned, to have been completed in a satisfactory manner. I agree that the amount set forth on this contract in the space labeled “Total” to be the total and complete charges for the work authorized and completed.
